Understanding Group Adult Foster Care (GAFC): Helping You Live Safely at Home
For many older adults and individuals living with chronic medical conditions or disabilities, remaining at home is the preferred choice. Group Adult Foster Care (GAFC) is a MassHealth program that makes this possible by providing essential daily support while helping individuals maintain their independence and quality of life.
At Best Home Health Care, we are proud to provide compassionate GAFC services that allow our clients to live safely and comfortably in the place they call home.
What Is GAFC?
Group Adult Foster Care (GAFC) is a Massachusetts MassHealth program designed for eligible adults who need assistance with activities of daily living (ADL) due to a medical condition, disability, or age-related limitations.
Unlike nursing home care, GAFC allows individuals to receive personal care and ongoing support while continuing to live in a community-based setting, such as their own home, an assisted living residence, or another qualified living arrangement.
What Services Are Included?
GAFC services are designed to support each person’s unique needs and may include:
Assistance with bathing, dressing, and personal hygiene
Help with mobility and transfers
Medication reminders
Monitoring changes in health and overall well-being
Care coordination with healthcare providers
Development of individualized care plans
Ongoing supervision and regular wellness visits
Every care plan is personalized to help clients remain as independent and comfortable as possible.
Who May Be Eligible?
You may qualify for GAFC if you:
Are enrolled in MassHealth
Need assistance with one or more activities of daily living (ADLs), such as bathing, dressing, toileting, transferring, or eating
Live in a qualified community-based setting
Meet the clinical and program eligibility requirements established by MassHealth
To help every individual live safely, independently, and with confidence in the comfort of their own home.
If you are unsure whether you qualify, our team is here to help you understand the eligibility requirements and guide you through the enrollment process.
The Benefits of GAFC
