What Is GAFC?

Group Adult Foster Care (GAFC) is a Massachusetts MassHealth program designed for eligible adults who need assistance with activities of daily living (ADL) due to a medical condition, disability, or age-related limitations.

Unlike nursing home care, GAFC allows individuals to receive personal care and ongoing support while continuing to live in a community-based setting, such as their own home, an assisted living residence, or another qualified living arrangement.

What Services Are Included?

GAFC services are designed to support each person’s unique needs and may include:

Assistance with bathing, dressing, and personal hygiene

Help with mobility and transfers

Medication reminders

Monitoring changes in health and overall well-being
