Adult Foster Care (AFC): Compassionate Support for Independent Living
Caring for a loved one is one of life’s most meaningful responsibilities, but it can also come with challenges. Adult Foster Care (AFC) is a MassHealth program that provides support for individuals who need assistance with activities of daily living while recognizing and supporting the family members or caregivers who make independent living possible.
At Best Home Health Care, we partner with families and caregivers to ensure every client receives compassionate, personalized care in the comfort and familiarity of their own home.
What Is Adult Foster Care?
Adult Foster Care (AFC) is a Massachusetts MassHealth program that supports eligible adults who are unable to safely live alone because of a chronic illness, physical disability, cognitive impairment, or other medical condition.
Unlike facility-based care, AFC allows eligible individuals to remain in their homes and live with a qualified caregiver while receiving the assistance they need with activities of daily living (ADL). The program also provides ongoing clinical oversight to support both the client and the caregiver.
Who May Benefit from AFC?
Adult Foster Care (AFC) may be a good option for adults who:
Need assistance with activities of daily living such as bathing, dressing, eating, or mobility
Have a chronic medical condition or disability
Require supervision due to memory loss or cognitive impairment
Prefer to remain at home rather than move into a nursing facility
Have a family member, friend, or other qualified caregiver who provides daily support
Our team works closely with clients and families to determine eligibility and ensure the program is appropriate for each individuals’s needs.
What Services Does AFC Include?
Through the Adult Foster Care program, Best Home Health Care provides comprehensive support designed to meet the unique needs of each client and caregiver, including:
Bi-Weekly tax free financial stipend paid to the caregiver
Regular visits from a registered nurse
Ongoing case management and care coordination
Individualized care planning
Health and wellness monitoring
Education and support for caregivers
Coordination with physicians and other healthcare providers
Assistance navigating available community resources
These services are designed to promote safety, independence, and overall well-being while helping caregivers feel supported in their important role.
Supporting Family Caregivers
Family caregivers are an essential part of the healthcare team. Adult Foster Care recognizes the value of their dedication by providing clinical guidance, ongoing support, and—when eligibility requirements are met—a financial stipend through the MassHealth program.
This support helps caregivers continue providing high-quality care while reducing stress and improving the overall care experience for everyone involved.
